Assumption of Risk/Waiver: Participant understands that during the event of Master’s Taekwondo Academy LLC’s instruction, physical contact between participant, instructors, or other persons will occur. Participant further acknowledges and understands that while reasonable precautions to avoid injury are taken, Injuries do occur, injuries may include, but are not limited to: bruises, sprains, damaged/dislocated/broken joints and bones, spine, ranging from minor to permanent and irreparable, to death. Additionally, participants may be exposed to human body fluids, including but not limited to blood. Therefore, there may be a substantial risk of exposure to communicable diseases, including but not limited to hepatitis and AIDS. With that full understanding, Participant freely and voluntarily agrees to assume the risk of injuries or disease associated with Master’s Taekwondo Academy LLC’s instruction and bear full responsibility for them by agreeing to release and /or waive any rights to claims for injuries or disease.

Arbitration of Claims: To the maximum extent permitted by Federal and State Law, Participant waives the right to a trial by jury and further agrees to submit all claims arising out of this Agreement and all claims the Participant may hereafter have against Master’s Taekwondo Academy LLC its employees, members, and event managers, agents to binding arbitration under the rules of the American Arbitration Association, in Kansas City, Missouri. Medical Release: In the event of an emergency, Participant hereby authorizes transportation and licensed medical personnel to perform any accepted medical procedures on the Participant deemed necessary or advisable, and agrees to bear the expense of any such procedure.
